Medicare Wellness
Once you are enrolled in Medicare, you are eligible to receive a Welcome to Medicare Wellness visit within the first 12 months. The goals of this visit are health promotion and screening. This visit includes the following:
- Review of medical and social history
- Screening for depression
- Screening for fall risk
- Blood pressure, height, weight, and BMI (this is not a hands-on exam)
- Discussion of end-of-life planning
- Education, counseling, and referral based on results of review
Inform the office when scheduling this appointment that this is your Welcome to Medicare visit to ensure proper billing.
Annual Medicare Wellness Visit
This annual wellness visit includes the following:
- Review and update of past medical, family, and social history
- Blood pressure, height, weight, and BMI
- Screening for depression
- Screening for fall risk and ability to perform activities of daily living
- Cognitive impairment screen
- Review of patient’s current medical providers, i.e. specialists involved in their care
- Review of medications
- Providing of health advice that includes patient’s risk factors, methods to decrease risk factors, and treatment recommendations
Inform the office when scheduling this appointment that this is your annual Medicare Wellness visit to ensure proper billing.
Your physician may recommend other services such as additional lab tests, immunizations, or imaging. Please check with your insurance provider about coverage for these services.
You may also have other concerns that you want to discuss or have addressed. These services may not be considered part of you preventive exam and may require an additional charge.
- New concerns such as back pain, abdominal pain, or wart/mole removal
- Current chronic conditions that need further evaluation, management, or treatment
